At Gracedale we have a short term rehab unit designed to get our residents back on their feet and safely back home in a timely manner.

Gracedale is committed to providing specialized care for our residents with Alzheimer’s disease as well as other forms of dementia.

Gracedale recognizes that sometimes our caregivers in the community need an opportunity to rest and reset in order to be the best caregiver they can be for their loved one.

Fill Out the Gracedale Application Form
Download the admissions application below to start. If you have questions, don’t hesitate to contact us. The completed application can be returned to the Admissions Department by email, fax, or U.S. Postal Service.
Along with the application, Gracedale requests financial, medical and social information. We’ll contact you to schedule an appointment, and we’ll arrange a facility tour once the application is complete and all information is provided.
If an appropriate bed is not immediately available, the applicant’s name will be placed on our waiting list.
Gracedale participates with Medicare & medicaid and most medicare advantage plans.
